On-Goal may include diabetes supplies, health coaching and/or medications and is based on your individual employer. Once the enrollment form is completed, our enrollment specialist will reach out to confirm your On-Goal program employer-provided benefits.
1. Diabetes Supplies
Whether you are newly diagnosed or have lived with diabetes for years, enrolling in the On-Goal program provides the resources, education, support, and supplies including test strips, lancets, lancing devices, blood glucose meters, and continuous glucose monitors (cgms).
2. Health Coaching
Helping members live healthier lives by managing their weight or diabetes through one-on-one health coaching! Our team of Licensed, Registered Dietitians provide you with the resources and motivation needed to reach your health and lifestyle goals.
3. Prescription Medications
Receive mail-order medication delivered to your door at no cost to you! The costs are covered by your employer’s health plan when you agree to regularly speak with a health coach. Save money while still maintaining your health!
